If you require any vaccinations and travel advice relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. Please call us as soon as you have sent in your pre-travel health questionnaire to book your appointment.
The questionnaire will ask which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 4 weeks before you travel. The Nurse will check your medical records and will offer travel advice even if you do not require any vaccinations or boosters.
**Please note that not all vaccines are provided by the NHS - the Nurse may advise you go to a private travel clinic for vaccines we are unable to give. This will incur a charge with the Private travel clinic and will be your choice.
Travel Health Questionnaire
Please complete a 'Pre Travel Questionnaire', print, complete and return it to the surgery as soon as possible (at least one week prior to your appointment). This enables the Doctor and Nurse to review your needs before your appointment with the Nurse. If you are unable to do this your travel vaccines may be delayed.
